EMPLOYMENT RELATIONSHIP: Just cause subject to probationary period
CURRENT HOURS AND STARTING TIME: Full-time (37.5 hours per week) position. The current starting time for this position is 8:30 a.m.
Under direction, provides direct services to program members, consumers, family members and designated others for the Vocational and Day Program Services (VDPS). Implements program activities and participates in research and program evaluation. Promotes consumer assessing of community-based resources. Facilitates team understanding as well as member assessing of community-based resources.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Conducts assessments, service planning, specialized intensive case management, case work, living skills, training, crisis intervention, advocacy, linking and monitoring for assigned clients.
Conducts home visits and performs home and community based interventions.
Completes Macomb County Community Mental Health (MCCMH) record keeping requirements, including clinical records such as Person-Centered Plans and case notes, as well as statistical reports.
Provides in-service training and educational activities to MCCMH staff, support professionals and members of the community.
Works cooperatively with members of other disciplines, agencies and community providers to ensure timely and accurate service provision.
Maintains and updates clinical knowledge by reading, in-service training in designated areas, and other activities determined in consultation with the agency’s annual plan and clinical supervisor, or designee.
Coordinates with family, aftercare programs, and ancillary support services to secure aftercare.
Participates in staff meetings, in-service training and supervision with a therapist.
Coordinates inter and intra-agency plans of service.
Monitors customer services through face-to-face consumer contact, facility visits, consultation with family members, significant others, service providers and records review.
Develops effective relationships with service providers, home staff and families.
Operates an automobile while performing assigned job duties.
Performs related duties as assigned.
Required Education and Experience
- A Bachelor’s Degree in Psychology, Social Work, Special Education, Counseling or other mental health related field from an accredited college or university
- Must meet qualifications as specified in the Michigan Department of Health and Human Services (MDHHS) Prepaid Inpatient Health Plan (PIHP)/Community Mental Health Services Programs (CMHSP) Qualifications per Medicaid Services as a Qualified Mental Health Professional (QMHP);
- Individual with specialized training or one (1) year of experience working with a person who has mental illness; and is a psychologist, physician, educator (with a degree in education from an accredited program), social worker, physical therapist, occupational therapist, speech-language pathologist, audiologist, registered nurse, therapeutic recreation specialist, licensed or limited licensed professional counselor licensed/limited-licensed professional counselor, licensed or limited licensed marriage and family therapist, or a licensed physician’s assistant; or
- A human services professional with at least a bachelor’s degree in a human services field
Preferred Education and Experience
- A Master’s Degree in Psychology, Social Work, Special Education, Counseling or other mental health related field from an accredited college or university
- A minimum of one (1) year experience working in a behavioral health setting
Required Licenses or Certifications
- Possession of required state licensure appropriate to position and ability to meet privileging and credentialing guidelines
- Must possess and maintain a current Michigan Chauffeur’s license within two (2) weeks from date of hire
